The original form that had the required information and was approved by DOH was not being used. Per interview, this new form was altered to make other required documentation easier. The discard column and more specific labeling of “time” to “time removed from temperature control” must be changed on the form.

Comments: TPHC forms were reviewed and found to be acceptable. The temperature was measured for items missing from the list and found to be acceptable. Thermometers were calibrated on site and found to be accurate. Three bay dishwashing was discussed. An employee sick foodworker policy was discussed. All cold and hot holding temperatures were found to be acceptable. The menu was reviewed at the time of inspection.

Comments: The spray wand at the dishwasher area drapes below the plane of the sink if it is not hung properly. It is recommended that it be properly maintained such that it doesn't hang that far down. Sinks in the kitchen are recommended to be labled with their intended purpose (i.e. handwash, wash, rinse, sanitize, food prep). Using Time as a Public Health Control was discussed for hot and cold potentially hazardous food on the serving line. Forms were not being maintained and per interview the manager was unsure it was still going to be used. If you are not going to use TPHC you must submit a formal written statement.
