Comments: Comments only: 1. Some beverages and three freezers are present in basement. According to Administrative Tribunal Docket #20150028, plans to properly finish the basement must be submitted to the Glens Falls District Office no later than June 30, 2015. To date, no plans have been submitted. In discussion with Carol, she is about to submit plans to use additional space upstairs and once approved, will move all beverages and freezers upstairs and out of the basement. The basement floor is unsealed concrete and is cracked. The ceiling is wood and is open. 2. Also mention in docket #20150028, all persons directly involved with food preparation must take an acceptable food service training course and upon completion submit the completion certificate to the Glens Falls District Office by March 31, 2015. In discussion with Carol, the previous cook took the course, but he no longer works for her. The new cook, Kristi Chase, has completed a Serve Safe course, but no proof has been submitted to our office. Must submit a copy of the new cook, Kristi Chase's service safe course completion certificate to the Glens Falls District Office as soon as possible. 3. According to metal stem probe thermometer, several food temperatures measured 45 - 46 degrees F inside the tall two sliding door True cooler. This cooler was packed with food leaving little air space. Recommend turning the cooler temp dial down to cold hold food below 45 degrees F. 4. In discussion with staff, the left bag of the three bay sink is used to wash produce. This bay is not equipped with an indirect drain, but the right side bay is. Provided education. Must use and designate the right side bay of the three bay sink for all food preparation. 5. Discussed preparation of cold salads, such as Tuna salad. Must use all pre-chilled (45 degrees F or less) ingredients when preparing cold salads. 6. Carol showed me the proposed walk in cooler and prep area. Must submit plans that include equipment
