Comments: A Time Table of Compliance Date of August 25, 2018 has been established to correct the violations listed above. A re-inspection will be conducted after this date to determine compliance. Facility is in compliance with state-wide waiver requirements, cooked potentially hazardous (TCS) foods were marked with discard time and Manager log sheets available. Discussed routine hand washing and proper use of plastic gloves and utensils with Manager during inspection. Sanitizer test kits (chlorine and quaternary ammonia) available for evaluating dish ware sanitizer and wiping cloth solution. Thermocouple available for evaluating hot potentially hazardous (TCS) foods, monitoring thermometers were available inside refrigerated storage equipment which is used to store cold phf's- were maintained at less than 45 dF.

Comments: Conducted a re-inspection due to previous violations noted on inspection report of August 7, 2017. Regarding Blue item #15 A- Kitchen prep area/ floors were cleaned and interior surfaces inside new walk-in freezer were checked- no ice build-up on floors, some ice build-up on ceiling surfaces- no frozen foods impacted. Spoke to maintenance man during re-inspection- will continue to monitor walk-in freezer for ice build problems.

Discussed stocking of potentially hazardous food (PHF) into refrigeration throughout facility in manner that prevents items reaching temperatures over 45F.

Comments: Violation 12c corrected from previous inspection. Freezer door has new seals, et cetera installed. Ice build-up in freezer noted on previous inspection has been greatly reduced. Discussed with operator watching for any ice build-up in the future and protecting food from ice as it is considered waste water. No other items evaluated at time of inspection.

The three cold-holding units (drink storage at drive-thru window, drink storage/milk cooler behind front counter, and smoothie/milk unit near drive-thru window) have thermometers. Observed that ice scoop is stored correctly in holder at ice bin for drive-thru window. Observed that facility is storing items in walk-in freezer such that food will not be impacted by ice build-up. Facility is under TTOC to replace walk-in freezer (6/1/15). Discussed with operator avoiding barehand contact with ready-to-eat foods (including ice and drinks). No other items evaluated at time of inspection.

Comments: Discussed with operator use of waiver for raw shell eggs and liquid eggs. Discard time must be recorded when item is taken out of cold holding. Walk-in freezer continues to have ice build-up on ceiling and around condenser drain lines. No food is stored under the condenser and drain lines. Replacement of walk-in freezer is scheduled for this spring and is under a TTOC of 6/1/15. TTOC for uncorrected items above is 1/19/15.

Comments: 8a corrected from previous inspection. Facility has re-arranged walk-in freezer so that no food is stored under condenser. Walk-in freezer is to be replaced next spring (TTOC 6/1/15). Problem has been lessened and food is not currently impacted. Facility to continue to not store food under condenser and to protect food from ice drippage.
