Comments: Management has prepared a spreadsheet to keep track of daily screenings of employees for symptoms. Spreadsheet will be emailed to WCHD daily until further notice. Only employees who have provided documentation of immunization for hep A will be eligible to work in food service, food prep or any job function whcih would involve being in the kitchens or possibly in contact with any food. Chlorine test paper is in use to verify sanitizer concentration is above 200 PPM. Designated employees are remaking sanitizer every 2 hours. Employees are wearing gloves at all times that they are handling, preparing, serving or otherwise possibly contacting food, a food prep surface or utensils. Wiping cloths are stored in sanitizing solution between uses. Handwashing is mandatory at the beginning of each employees shift.

Comments: Re-inspections to be conducted frequently to ensure continuation of all precautionary measures. Facility management is instructed to continue to follow all precautionary practices documented in report completed yesterday 11/10/2017, including: -handwashing by scrubbing all parts of hands and wrist with soap and hot water and dry with paper towels. -handwashing to occur at beginning of shift, throughout day and after using bathroom, eating drinking or touching face. -gloves to be worn by all food workers and servers at any time when they are preparining food, serving food, or are in or near a food prep area. -sanitizing solution buckets to be emptied and refilled with freshly mixed sanitizer every 2 hours. Operator has placed a notice to all employees throughout food prep areas including above time clock used for punching in: -handwashing -sanitizer usage -glove usage -daily interview by supervisors at the beginning of each shift to ensure that employees are not experiencing any symptoms of hepatitis A. Additional precautionary practices to be insituted at this time: -employees will be restricted from bringing their cell phones into the bathrooms -EZ serv snack dispenser in basement to be sanitized -caddie soda vending machine surfaces and buttons -ice scoops will not be stored in ice. ice will only be scooped by employees wearing gloves using a scoop with a handle -dry wiping cloths used as pot holders will be sent to laundry after use -computer stations to be sanitized including desks, keyboards, monitors and adjacent areas and writing utensils Facility has permission to begin food prep at this time and to begin serving food tommorow at 11 AM.

Comments: Met on site with general Manager Eric Rule and Director of Culinary Operations and Executive Chef Jack Kiggins. Inspected kitchens, dining areas, bars, dining areas and temporary residence pantry. Provided instruction in all staff meeting including: -sanitization procedures (including sanitizer preparation and wiping cloth storage) -use of gloves and avoidance of barehand contact on ready to eat food -reviewed proper handwashing procedure including scrubbing thoroughly all parts of hands and wrist with soap, rinsing with hot water, and drying with single service, disposable paper towels. Operator agrees to institute the following precautionary measures until otherwise instructed by this department: -all employees involved in food service must be vaccinated prior to returning to work -all employees will be interviewed by a designated staff member prior to each shift to affirm that they are not experiencing any symptoms of Hepatitis A -symptomatic employees will be restricted from work until they provide documentation of a negative HepA test and receive clearance from WCHD Disease Control Staff -until further notice from this department, gloves to be worn by all staff preparing, handling or serving food. Gloves will be worn by all employees with any chance of incidental contact with food, food prep areas and surfaces, or utensils. -Workers who are confirmed to have been vaccinated PRIOR to this incident will be used as much as possible for the majority of food prep assignments. -Workers who were vaccinated DURING this event will be assigned to food prep only when adequate previously vaccinated workers are not available. -Food service to the public may not commence until the satisfactory completion of a pre-operational inspection by this department.
