Comments: One potentially hazardous pizza containing cooked broccoli was being stored on counter at room temperature and was temped using a metal stem-probe thermometer to be 76°F. Time was being tracked with a printed out receipt and pizza had been cooked within the hour of inspection, but facility does not have a TPHC waiver. TPHC waiver paperwork was provided and explained to operator over the phone. Operator stated that staff discards potentially hazardous pizzas after four hours of being cooked. Please submit TPHC waiver paperwork to NYS DOH Glens Falls District Office within two weeks. All violations must be corrected and are subject to enforcement action if not addressed in a timely manner.

Operator must inform NYSDOH which accountability system will be used by 5/1/15. *Mechanical dishwasher must be serviced by 5/1/15, and invoice submitted to NYSDOH. Dishwasher must either reach 180F on the final rinse cycle, or inject the attached sanitizer (chlorine bleach) such that a residual of 50ppm is left on the dishes. Ensure that test strips are on site and available to evaluate the concentration of bleach left on dishes. *Floors (kitchen, walk-in cooler) to be repaired by 8/1/15. *All other violations to be corrected by 5/15/15.
